SCIENCE LABORATORY SAFETY RULES
(This form may be printed and returned to the teacher.)

    The science laboratory is an enjoyable, exciting place to work or learn.  It must also be a safe place.  I agree to conduct myself well while in the lab.  I agree to follow my teacher's instructions at all times and follow the safety rules listed below:

  1. I will always get my teacher's permission before beginning a lab activity.
  2. I will never run, push, or play in the lab.
  3. I will never eat or drink anything in the lab.
  4. I will always use care when working with unknown substances.

    5. This means:
                        * I will never touch anything without the teacher's permission.
                        * I will never smell any substance without my teacher's permission.
                        * I will never taste any substance without my teacher's permission.
  5. I will always use care when I handle lab equipment.

    6. This means:
                        * I will be careful with glassware.  I report all chipped or cracked
                            glass
            to the teacher.
        * I will report any glass breakage to the teacher.
        * I will learn the location and proper use of the safety equipment in
            the lab.
  6. I will always report any accident to my teacher immediately.

RETURN TO THE TEACHER SOON:  ALL STUDENTS MUST HAVE A SIGNED SAFETY FORM ON FILE TO PARTICIPATE IN THE LAB ACTIVITIES.
